Our office hours are by appointment only, and we only schedule appointments over the phone, to make sure we have plenty of time to answer your questions.

After we schedule your first appointment, we will send you information in the mail. You will not need to complete any paperwork prior to your visit, but you will be asked to sign a consent form when you arrive, so that Dr. Papak can see you, provide care, and file your visit with insurance.


For Your First Office Visit, Please Bring:
  • your insurance card/s
  • a photo ID
  • your copay if applicable
  • a written or printed copy of your medication list
We do not need your medicine bottles, and we cannot copy a list from your phone, but looking at your medication list can tell us a lot about your health history and help us tailor your foot care.

Our office does not accept any forms of Medicaid. If you have insurance other than Medicaid, Dr. Papak will likely be able to tell you at your first visit if future visits will be covered by your insurance. If not, we offer very reasonable pricing for out-of-pocket payments. Many of our patients will tell you that not having to worry about working on their own toenails is priceless!

If you have questions or concerns, call us anytime. Office voicemail messages go directly to Dr. Papak, and all return calls are handled either by her or by Kim, a licensed practical nurse.
